TURNAROUND HSE EXCELLENCE

Executive summary

Turnarounds compress an exceptional volume of non-routine work into a short period in high risk facilities. Plant configuration changes rapidly, thousands of people may be mobilised, contractor interfaces multiply, and high-risk work is executed simultaneously under intense schedule and production pressure. The result is not simply “more work”. It is a different operating environment in which normal supervision capacity, permit systems, learning routines and management attention can be overwhelmed.

Industry guidance recognises shutdowns, outages and turnarounds as particularly demanding because disparate repair and upgrade activities must be completed in a compressed period, often by specialist and transient contractor workforces. It highlights work control, supervision and competence as recurring success factors.[1] Regulatory process safety requirements similarly place explicit duties on owner organisations to evaluate contractor safety performance, communicate process hazards, establish safe work practices and periodically assess contractor performance.[2]

The central challenge is therefore not solved by adding more paperwork. Successful turnaround HSE performance requires the organisation to simplify how critical risks are controlled, establish sharper accountability for contractor readiness and performance, and place decision-quality risk controls as close as possible to the work.

This paper presents a practical approach built around three principles—Reduce Complexity, Sharpen Accountability and Move Risk Control to the Frontline—and illustrates how those principles were applied across the full lifecycle of an anonymized “Mega-Turnaround”. The implementation achieved zero Serious Injuries and Fatalities (SIFs) while generating actionable lessons for the design of future events.

1. Why turnaround HSE needs its own lens

A turnaround is not routine operations at higher volume; it is a temporary operating system with a higher risk profile.

During normal operations, organisational roles, work patterns, plant conditions and workforce composition are relatively stable. A turnaround changes all four at once. Equipment is opened, isolated, cleaned, inspected, repaired and recommissioned. Temporary facilities and logistics routes are introduced. Contractors with different systems and experience levels work side by side. Leaders must make decisions with incomplete information while the schedule continues to move.

This creates a distinctive risk profile. Permit-to-work processes can become congested. Supervisors may be responsible for more work fronts than they can meaningfully verify. Toolbox Talks and task risk assessments can drift toward procedural completion rather than risk anticipation. Contractor capability and welfare problems can appear only after the workforce is already on site. Leadership presence may increase yet still fail to concentrate on the activities where exposure and uncertainty are highest.

At peak, approximately 1,300 permits were issued per day. Fourteen contractors performed the majority of high-risk work, while multiple contractors and vendors operated simultaneously across the facility. The event therefore required a different HSE approach: one designed to maintain control quality when conventional systems were exposed to extreme demand.

2. Five recurring failure patterns

The most visible issue may be an unsafe act or permit delay, but the underlying failure is often systemic overload.

Turnaround risk is not evenly distributed across the event. It often follows a “bathtub” pattern: exposure is higher during initial mobilisation and ramp-up, decreases as teams and routines stabilise, and rises again during close-out and recommissioning as fatigue, schedule pressure and changing conditions accumulate.

These patterns reinforce one another. Permit congestion consumes supervisory attention. Reduced supervision allows task-level controls to become routine. Weak contractor readiness creates late changes in people, equipment and work methods. Leadership then spends time reacting to visible deficiencies instead of anticipating where the risk profile is moving.

Equip supervisors, foremen and workers to recognise, verify and act on risk at the point of work through a simplified set of Life-Saving Rules.

Principle 1 — Reduce Complexity

Simplification is not the removal of controls. It is the deliberate removal of duplication, unnecessary handoffs and competing information so that critical controls remain visible. In our case study, Permit-to-Work grouping reduced the projected permit volume from approximately 48,000 to approximately 30,000. The turnaround HSE plan was simplified to prioritise control over paperwork, and TA-specific training was designed around the actual risk profile of the event.

The test is straightforward: under peak conditions, can a supervisor understand what is changing, which controls are critical and where intervention is required? If the system cannot answer those questions without extensive searching or reconciliation, complexity has become a safety risk.

Principle 2 — Sharpen Accountability

Turnaround contractor management must extend beyond qualification and site access. Contractors need explicit ownership for closing capability, mobilisation and field-verification gaps. The owner organisation must maintain a clear line of sight from assessment findings to named actions, deadlines and escalation decisions. In the case study, contractor-specific gap closure plans, pre-mobilisation checks, mobilisation assessments, site verification and consolidated on-site audits created a progressive assurance chain rather than a one-time entry gate.

Principle 3 — Move Risk Control to the Frontline

The HSE function cannot directly control thousands of simultaneous decisions. Frontline supervisors, foremen and workers must be able to identify abnormal conditions, verify controls and stop or adapt work. The case study strengthened Life-Saving Rule application through targeted learning campaigns, dedicated Visible Safety Leadership training for frontline supervisors, foremen and selected contractor supervisors, senior leadership engagement and joint HSE walkabouts.

4. Apply HSE across the full turnaround lifecycle

Late intervention produces late risk visibility. Turnaround HSE must begin when strategy, scope and contractor decisions are still changeable.

The intervention should mature as the turnaround progresses. Early phases define governance, risk appetite and the criteria for contractor selection. The middle phases convert the scope into work packs, resource plans, competence requirements and mobilisation controls. Execution then requires a tight operating rhythm for permit control, field assurance, change management and rapid learning. Post-turnaround learning must feed directly into the next event rather than remain as an isolated close-out report.

This lifecycle approach avoids a common failure: treating HSE as a mobilisation or execution workstream after the commercial, schedule and scope architecture has already been fixed. By that point, many of the most important risk controls—contract strategy, supervision ratios, logistics design, work packaging and sequence—are expensive or impossible to change.

The following case study shows how this lifecycle approach was translated into a risk-based contractor assurance model.

5. Case study: contractor risk concentrated, assurance targeted

Risk-based segmentation helped direct limited assurance capacity to the contractors and activities with the greatest potential exposure.

Fourteen high-risk contractors represented approximately 85% of the turnaround workforce. This concentration created an opportunity: rather than apply the same level of assurance to every contractor, the programme focused deeper assessments, targeted audits and action tracking on the contractors most likely to influence the overall risk profile.

The assurance sequence began with HSE qualification and capability assessment (See our related thought leadership paper for a detailed overview of the Contractor HSE Qualification process), followed by contractor-specific gap closure planning. Pre-mobilisation and mobilisation reviews tested whether the contractor had converted commitments into resources, people, equipment and workable arrangements. Site verification then checked the reality of execution. This progressive model helped reveal issues early enough to correct them before the highest-risk phase of the event.

Readiness should be treated as a moving state, not a one-time score

Contractor readiness changes as personnel, supervision, equipment, scope and site conditions change. A capability assessment provides an initial view, but it cannot replace verification during pre-mobilisation, mobilisation and field execution. In the case study, average completion of pre-mobilisation and mobilisation requirements reached 72%, while site verification provided an additional reality check against documented readiness.

The most important output was not the score itself. It was the visibility of what remained incomplete, who owned closure and whether the gap could compromise safe execution. This is the difference between contractor assessment as compliance and contractor assessment as operational risk management.

Contractor readiness, however, was only one part of the challenge. The next requirement was to translate critical controls into consistent decisions across thousands of frontline interactions.

6. Strengthening frontline execution through simpler rules and visible leadership

The turnaround combined Make Compliance Easier, Life-Saving Rules and Visible Safety Leadership to translate HSE expectations into practical frontline control.

Under the Make Compliance Easier approach, requirements were simplified around the most critical risks and supported by a clear and proportionate consequence-management approach. A simplified set of nine Life-Saving Rules created a common control language for Bypassing Safety Controls, Confined Space, Driving, Energy Isolation, Hot Work, Line of Fire, Safe Mechanical Lifting, Work Authorization and Working at Height.

The Life-Saving Rules were reinforced through training, digital learning and leadership-led communication. During the turnaround, training was delivered to 8,798 contractors and 1,450 company employees. Online LSR training was assigned to 3,900 employees. The cascade was further supported through sessions attended by 83 leaders, communication delivered by team leaders to 841 employees, and contractor-focused training reaching 580 employees across 20 companies.

The Visible Safety Leadership approach complemented the cascade through both targeted field engagement and frontline leadership training. Dedicated VSL training was delivered to frontline supervisors and foremen and extended to selected contractor supervisors, strengthening their ability to recognise risk, verify critical controls, intervene when conditions changed and reinforce the Life-Saving Rules during daily work. Senior leaders and contractor management also participated in targeted joint HSE walkabouts, while Start Strong and Finish Strong campaigns helped sustain attention throughout the turnaround period.

7. What the Mega Turnaround taught us

Zero SIFs is an important outcome, but learning quality determines whether the result can be repeated.

These interventions strengthened control during execution, but zero SIFs did not mean that every control performed as intended. Learning quality determines whether the result can be repeated. The case study still identified vulnerabilities across work authorization, contractor management, isolation and LOTO, lifting, site conditions, welfare, safety rounds and incident response. These lessons show why turnaround learning must examine weak signals and operational friction—not only recordable events.

The lessons also raised a practical question: how can the same discipline, visibility and learning rhythm be repeated at scale without adding further administrative burden?

8. Digital enablement: make readiness and learning scalable

Digitalisation should reduce operational bureaucracy and administrative burden—not create another system that frontline teams must work around.

The next stage of Turnaround HSE Excellence is to digitise the operating rhythm that supports the three principles.

Simplify contractor management workflows

  • Digitise capability, pre-mobilisation, mobilisation and site-verification assessments within one workflow.
  • Make action ownership, evidence, deadline and escalation visible to both the owner organisation and contractor.
  • Reduce email and telephone coordination by providing a single, transparent view of requirements and status.
  • Use risk-based segmentation to focus assurance on the contractors and work scopes that matter most.

Reach the frontline through a simplified channel

  • Deploy short, role-specific micro-learning for Life-Saving Rules, alerts, lessons learned and changing risk conditions.
  • Use reminders, checks and brief quizzes to reinforce understanding instead of relying on one-off classroom attendance.
  • Capture frontline signals and questions so that management communication is informed by what is happening in the field.
  • Scale consistent learning to thousands of workers within minutes or hours, while preserving traceability.

Digital workflows can strengthen visibility and execution, but they cannot define risk appetite, assign accountability or make intervention decisions. Those remain leadership responsibilities.

9. Leadership and governance implications

Turnaround HSE performance is a line-management outcome supported by HSE expertise—not an HSE department deliverable.

Executive leaders define the risk appetite, protect readiness gates and ensure schedule or production pressure does not silently rewrite the control strategy. Turnaround management integrates HSE into scope, sequencing, resource and contractor decisions. Middle managers convert the plan into clear accountabilities and escalation rules. Frontline leaders control the work, verify conditions and respond to change.

Visible leadership is most effective when it is intentional. Visits should be directed toward high-risk work, changing conditions, weak contractor readiness, repeated permit problems and areas where frontline teams need decisions—not simply toward locations that are easy to access or already performing well.

Questions leaders should ask before the next turnaround

  • Which activities and contractors will create the greatest concentration of serious-injury and process-safety exposure?
  • Which controls can be simplified without weakening the risk barrier?
  • What evidence must a contractor provide before mobilisation—and what will be verified in the field?
  • How will contractor gaps be assigned, escalated and closed before work starts?
  • Which frontline decisions require better information, competence or stop-work authority?
  • How will leadership presence be targeted using the live risk profile?
  • What weak signals will be reviewed daily, even when no recordable event has occurred?
  • How will learning from this turnaround be built into the strategy, scope and contracts for the next one?
  • Where can digital workflows remove complexity, duplication or fragmented communication and foster the frontline-reach?

10. Conclusion

The best turnaround HSE systems are disciplined, risk-led and deliberately simple.

Turnaround HSE excellence is not achieved through a larger rulebook or a higher volume of HSE activity. It is achieved by designing a temporary operating system that remains effective under pressure. That requires the organisation to reduce complexity, sharpen accountability and move risk control to the frontline.

The anonymized case study demonstrates how those principles can be applied across strategy, readiness, mobilisation, execution and learning. Permit workload was reduced, high-risk contractor assurance was targeted, accountability was strengthened and leadership activity was aligned more closely with exposure. The event achieved zero SIFs, while the post-event review identified important opportunities to improve work authorization, supervision, isolation, lifting, site conditions and workforce welfare.
