From Field Presence to Frontline Ownership

Executive summary

Visible Safety Leadership is often mistaken for more site visits, more observations or more senior management visibility. Of course, being in the site is valuable, but the real value is: it helps leaders see how work is performed, understand the conditions that shape behaviour, and act on weak signals before they turn into incidents.

To strengthen VSL, organisations need to move beyond checking compliance and create better safety conversations in the field. Leaders should use their presence to ask elevating questions, listen to frontline experience, remove system barriers and reinforce the behaviours that make compliance easier.

This is also where a frontline-focused Behavior-Based Safety programme becomes important, It creates a structured system for increasing HSE awareness, building local safety champions and translating leadership intent into daily peer-to-peer safety behaviours.

The UK Health and Safety Executive note that human failure contributes to almost all accidents and exposures to hazardous substances, and cites major accidents such as Texas City, Piper Alpha and Chernobyl as examples where human failure-initiated events. The lesson for VSL is not that people are the problem; it is that leaders need better ways to understand the conditions that shape behaviour, close the gap between planned work and real work, and surface weak signals early. 1. Why Visible Safety Leadership needs to be more than visibility

In high-risk industries, leadership is part of the safety control system. The way leaders prioritise work, respond to bad news, talk about risk and act in the field tells people what really matters. A field visit can therefore either reinforce learning and trust or quietly confirm that production pressure is still stronger than safety expectations.

The strongest VSL practices are not built around counting visits. They are built around the quality of the interaction. The difference is simple: a visit that only checks whether people follow the rules may confirm compliance, but a conversation that explores how the work is being done can reveal risk that no dashboard or procedure can fully show.

This is why the leadership question should shift from “Have we been visible enough?” to “Did our visibility help the organisation learn something useful and act on it?”

2. Leadership signals shape organisational behaviour

Every leader sends signals. Some are obvious: what is discussed first in meetings, which actions receive resources, how stop-work decisions are treated, and whether safe behaviours are recognised. Others are subtle: rushing a toolbox talk, reacting defensively to a concern, or accepting a workaround because the job needs to finish.

People read these signals quickly. If leaders ask thoughtful questions, listen actively and respond fairly when things go wrong, teams learn that safety is a shared responsibility. If concerns are downplayed or learning is replaced by blame, teams learn something different: stay quiet, avoid attention and keep the work moving.

The practical test of VSL is therefore not whether leaders say safety is important. It is whether their daily signals make it easier for people to speak up, pause, challenge, clarify and choose the safer path under pressure.

3. Work as Imagined vs Work as Done – Normalisation of Risk

Leadership signals become powerful when they are directed at real work. Procedures, risk assessments and permits describe how work is expected to happen. Field engagement shows how work is actually done under changing conditions, competing priorities and local constraints.

Work as Imagined is the organisation’s view of the task: the planned sequence, the documented controls, the expected resources and the formal rules. Work as Done is the lived reality: what people do when equipment is not ideal, information is incomplete, weather changes, time pressure increases, or contractor interfaces create uncertainty.

The gap between the two is not automatically a violation. In Safety-II thinking, a modern safety perspective focused on learning from normal work and successful adaptation, Work as Imagined and Work as Done help explain why formal plans and actual performance can diverge under changing conditions. Often, this gap is where frontline teams adapt intelligently to keep operations running. The risk is that these adaptations can become invisible to leaders. When people live with the same gap every day, it can start to feel normal — “this is how the job is done here.” This is where risk normalisation begins. (Hollnagel, Wears and Braithwaite)

A mature VSL approach treats the frontline as a source of operational intelligence. Leaders go to the field not only to verify standards, but to understand where standards, resources and reality no longer fit together. The critical question is not only “Is the procedure being followed?” but “How do we know where the gap is in our organisation, and what are we doing about it?”

This requires leaders to ask escalation questions that move the conversation from observation to action:

  • Where does the gap between planned work and actual work show up most often?
  • What makes this workaround or adaptation feel normal?
  • What conditions would make this gap unsafe?
  • Can the team close the gap locally, or does it require management action?
  • Does the procedure need to change, or do resources, planning assumptions and expectations need to change?
  • Who needs to know, and how will we close the loop with the team?

In this way, VSL helps prevent risk from becoming normalised. It makes hidden adaptations visible, turns frontline experience into leadership insight, and creates a pathway for action before weak signals become incidents.

4. Human Performance: moving beyond human error

Once leaders understand that real work is variable, “human error” becomes a starting point for learning rather than an endpoint for blame. Many incident reports describe what a person did wrong. Fewer explain why that action made sense in the context at the time.

Human Performance thinking helps leaders ask better questions. IMCA’s HOP guidance summarises the practical principles clearly: human error is normal; blame fixes nothing; context drives behaviour; learning is vital; and the leadership response to failure matters. For VSL, this means looking at workload, equipment condition, supervision, team norms, procedures, incentives, fatigue, communication and production pressure before reaching conclusions about individual behaviour (IMCA). This represents a significant paradigm shift for high-risk industries. Instead of trying to “fix” individuals, the focus moves toward designing work environments where people can fail safely. The question is not whether people will make mistakes; they will. The real leadership question is whether the system allows those mistakes to be detected, contained and learned from before they lead to harm.

The commonly cited message that a large share of incidents involves human error should therefore be handled carefully. Used badly, it becomes a reason to blame people. Used well, it becomes a reason to improve the system around people.

For VSL, the leadership response matters most. A defensive response teaches people to hide weak signals. A curious and fair response teaches people to report, reflect and learn. This is how field conversations begin to strengthen barriers instead of simply documenting deviations.

5. Psychological Safety with high standards

Human Performance creates a better way to interpret behaviour, but people still need the confidence to speak honestly. In many operations, the most important information is held by the people closest to the work: small deviations, awkward workarounds, informal shortcuts, unclear instructions and early signs of barrier weakness.

Psychological Safety is what allows that information to surface. Amy Edmondson is clear that Psychological Safety does not mean lowering standards, avoiding accountability or accepting poor performance. Its value comes when it is combined with high standards: people are expected to perform, but they can also speak up, challenge assumptions and admit uncertainty without fear of humiliation or punishment. This balance is critical. High accountability without Psychological Safety can push teams into anxiety: people comply on the surface but may withhold bad news. Psychological Safety without accountability can create comfort without discipline. VSL should aim for the Learning Zone: clear expectations, honest conversations and visible follow-through.

For leaders, the maturity of Psychological Safety is not measured by how comfortable people say they feel, but by what they are willing to raise when the message is difficult. Do people challenge unsafe assumptions? Do they report weak signals early? Do they admit uncertainty before decisions are made? Do they feel able to stop, pause or clarify when work conditions change?

The fine line between performance and Psychological Safety is managed through clarity. Leaders should be clear about standards, risk tolerance and expected behaviours, while also making it safe to discuss gaps, mistakes and concerns. In mature safety cultures, speaking up is not treated as resistance to performance; it is treated as part of how performance is protected.

6. Shining Star: making VSL practical at the frontline

A strong Visible Safety Leadership approach should work at both leadership and frontline levels. Senior leaders create direction, set expectations and send visible safety signals through their decisions, language and field presence. Frontline teams, however, are where safety culture is experienced every day: in shift handovers, toolbox talks, peer interactions, contractor interfaces and decisions made under real operational conditions.

This is also a competency question. As discussed in our series, safety capability should not be treated as a training record or a generic awareness exercise. It needs to be built around the roles, behaviours and moments that materially influence risk. In VSL, this means developing not only leaders who can engage the field, but also frontline people who can recognise risk, challenge normalisation, support peers and coach safe behaviours in real work.

This is why VSL should be supported by a frontline layer that turns leadership commitment into daily safety behaviours. The Shining Star Programme provides this layer. It is a frontline-focused Behavior-Based Safety development system designed to increase HSE awareness, build local safety champions and strengthen safe behaviours through observation, feedback, coaching and peer-to-peer influence.

A key feature of the programme is its structured development model. Participants progress through levels such as Bronze, Silver and Gold, supported by selected coaches who guide, validate and reinforce their field activities. This creates a measurable frontline development pathway and a multiplier effect: safety leadership is strengthened not only through senior leaders’ field presence, but also through trained frontline champions and coaches who keep safe behaviours visible in daily work.

A recent frontline implementation at one of our client’s facility illustrates how this model can move from concept to measurable progress. The programme engaged 50 frontline specialists, supported by 10 active coaches, with a balanced participation across operations and maintenance roles. Internal tracking showed a 92.7% average completion rate and 649 completed tasks across defined safety activity criteria.

The value of this data is not only the completion rate. It shows that frontline development can be structured, coached and measured. Participants were not simply attending awareness sessions; they were completing practical field activities linked to toolbox talks, safety moments, shift handovers, observation reporting, emergency drills, PTW, root cause analysis, process safety induction, major accident hazards, MOC and action management.

The programme also demonstrated how capability can mature over time. Bronze level established the foundation: individual participation, safety awareness and completion of core field tasks. The Silver level then shifted the emphasis from practitioner to team leader, expanding the scope from personal performance to coaching, system ownership and delivery of measurable team outcomes. This staged progression is important because it turns frontline safety from a one-off engagement activity into a visible capability-building pathway.

In practice, the program connects directly with the earlier VSL themes. It helps frontline teams make Work as Done more visible, apply Human Performance thinking in daily conversations, and raise weak signals before they become incidents. Participants are encouraged to observe real work, recognise safe and unsafe behaviours, ask better questions, provide constructive feedback and escalate barriers that cannot be solved at team level.

The expected value is not simply more observations or more safety conversations. The real value is stronger frontline ownership. Shining Star gives frontline teams a structured way to contribute to safety leadership, while the top management layer of VSL ensures that expectations, resources and leadership responses remain aligned.

What Shining Stars should enable

  • higher HSE awareness through structured frontline development;
  • visible local safety champions who influence their teams through daily behaviours;
  • better quality safety observations and peer-to-peer feedback;
  • stronger coaching capability to support frontline development;
  • earlier escalation of weak signals, local barriers and recurring workarounds;
  • a practical bridge between leadership commitment and frontline reality;
  • measurable progress through staged development levels.

In this sense, Shining Star strengthens VSL by making it operational at the frontline. Top management creates the direction, expectations and conditions; frontline champions help turn them into daily practice. The next leadership question is therefore practical: what should leaders do differently to support this system and sustain frontline ownership?

7. Digital layer: making field signals visible

Shining Star helps strengthen frontline ownership, but a mature VSL system also needs a way to capture, connect and act on the signals emerging from daily work. Leaders may ask better questions, and frontline champions may raise better observations, but the organisation still needs a practical mechanism to see patterns, measure maturity and close gaps before incidents occur.

This is where digital tools can strengthen, rather than replace, safety leadership. The purpose of digitalisation is not to create more reporting burden. It is to help leaders at every level understand what is happening in the field, where behaviours are changing, where weak signals are emerging and where support is needed.

8. Closing message: connection over control

Visible Safety Leadership is not a programme of field visits. It is a leadership discipline that helps organisations understand real work, improve the conditions that shape behaviour and create the trust required for people to speak up before risk becomes harm.

The strongest safety cultures are not the ones where people never make mistakes. They are the ones where people can surface reality early, learn quickly and improve the system before failure becomes visible or a place where people can fail safely.

This is where the combination of top management VSL and frontline Shining Star capability becomes powerful. Leaders create the direction, expectations and conditions for safe work; frontline champions help turn those expectations into daily behaviours, peer influence and local ownership.

In that sense, VSL is strengthened when visibility becomes learning, learning becomes action, and action becomes shared ownership at the frontline — supported by digital systems that help the organisation see, measure and respond to weak signals before they become incidents.
